Tight Waistbands Are Giving South Florida Adults Intertrigo

If you've ever pulled off your jeans at the end of a long, sweaty day and found an angry, raw patch of skin underneath your waistband, you're not imagining things. That's not just "chafing." There's a good chance it's intertrigo — and it's one of the most common things we treat at Dermatology Experts, especially once the humidity kicks in.
**What's actually happening under there**
Intertrigo happens when skin rubs against skin (or against tight fabric) in warm, moist areas of the body — think waistbands, under the belly, underarms, and inner thighs. Add South Florida's heat and humidity to the mix, and you've got the perfect environment for skin to break down, get irritated, and then get colonized by yeast or bacteria on top of it. What starts as simple friction can turn into a red, sometimes oozing, often itchy or burning rash within days.
Tight waistbands are a huge culprit here. Shapewear, compression leggings, and skinny jeans trap sweat against the skin all day, and if you're sitting at a desk or in a car with AC blasting on the outside while your waistband cooks things on the inside, that skin never gets a chance to dry out. We've seen this same pattern show up in patients dealing with tight activewear and compression leggings, and it's exactly why intertrigo shows up more in our Miami, Parkland, and Tamarac patients than in drier climates.
**How we diagnose it (and why the code matters)**
When you come in with this rash, we're not just eyeballing it and sending you on your way. Intertrigo has its own diagnostic code — ICD-10 L30.4 — which matters for your insurance and for making sure we're treating the right thing. That's important because intertrigo can look a lot like other skin folds issues, including fungal infections, bacterial infections, or even a flare of eczema that's settled into a skin fold. Getting the diagnosis right the first time means you're not wasting weeks on the wrong cream.
**Intertrigo treatment that actually works**
Treatment depends on what's going on underneath the irritation. If it's pure friction and moisture without infection, we'll usually start with:
- A barrier ointment or zinc oxide cream to protect the skin and let it heal
- Drying powders to keep the area moisture-free throughout the day
- Looser, breathable clothing recommendations (yes, we will tell you to size up)
If yeast or bacteria have moved in — which happens often in our climate — you'll likely need a prescription-strength intertrogo cream with an antifungal or antimicrobial ingredient, not just an over-the-counter diaper rash cream. This is the part patients get wrong the most: grabbing whatever's in the drugstore aisle without knowing what's actually causing the irritation. The wrong cream can make things worse or just mask the problem while it keeps spreading.
For patients dealing with recurring flare-ups, we'll also talk through daily habits — how you're drying off after showers, what fabrics you're wearing, whether you're sitting in sweaty clothes post-workout. If this sounds familiar, it's worth reading about how gym culture is spreading skin infections across South Florida, since the same damp-fabric problem applies here.
**When it's more than just a rash**
Most of the time, intertrigo clears up fast once you get the right treatment. But if the rash keeps coming back no matter what you do, or if it's spreading, oozing, or incredibly painful, it's worth getting it looked at in person. Some patients dealing with chronic friction and inflammation in skin folds actually have an underlying condition like hidradenitis suppurativa, which needs a completely different treatment approach.
The good news: this is one of the easier things we fix. You don't need to live with that burning, raw feeling every time you put on real pants. Come see us, we'll figure out exactly what's going on, and we'll get you a plan that actually works — not another guess from the pharmacy shelf.
